Our process begins with an interview with the patient’s family, where we establish goals and address concerns regarding the care currently being received by the patient. This interview is followed by a separate interview with the patient, which can also be attended by family members, where we introduce ourselves to the patient and discuss the patient’s goals and concerns regarding care.
